Due to a discussion on the google group, I decided to start a thread here, to keep things semi-permanant and searchable:

Lauren was asking about preping some emegerency procedures, such as contact info, local hospital (and if possible, a map, and English speaking contact there), etc

Particularly:
Police 110
fire/ambulance 119
Japan Helpline (24/7, English, toll free) 0570 000 911
Tokyo English Life-Line (9-11pm) 03 5774 0992

I would assume either of those will relay/translate between THS and police/fire/ambulance services in a pinch.

I suggest that whatever we come up with, it should have our address and the emergency numbers on one side, and a map to the local hospital on the other.

In addition, it absolutely must be laminated.

We also need to prep an earthquake kit, and proper emergency medical kit, and be sure everyone knows where it is.
